REVOCATION PROCESS:

Driving is a privilege! FOLLOW THESE STEPS AS DIRECTED!

Arizona Revocation Steps

Determine Eligibility for Arizona Revocation:

1. Send Extended Certified MVR and Photo ID to [email protected]

2. If you’ve had a DUI after 2010, send ALL completed treatment Certificates.

3. If you do not have proof of treatment, completion of 36 hours of DUI treatment is required to proceed.

 

After Determination for Eligibility:

1. Complete AZ Revocation Application

2. Pay $125 to CK Counseling group for Arizona Revocation Evaluation.

3. Complete Questionnaire

4. Complete Screeners/Consents in Portal

5. 30 min Evaluation for Further Screenings and Final Approval

16 HOUR DUI EDUCATION ONLINE

To successfully complete your 16-hour DUI Education course, you will need an internet connection and a smart device/computer. Additionally, you must provide proof that you require the 16-hour course. This can be accomplished by submitting a copy of your screening note (if the screening was completed with another facility), MVD correspondence confirming your eligibility for the interlock deferral program (SIIRDL), or court documents. Lastly, a state-issued ID copy is also necessary. With a photo ID copy and proof of the need for the 16-hour course, reporting to the courts and MVD will be completed on time.

DUI EDUCATION PROCESS:

  • Pay for the 16 Hour DUI course
  • Complete your user profile
  • If you have completed a screening with an facility upload the Screener’s notes into your dashboard
  • Access your 16 Hour DUI Education through your user dashboard
  • Complete your online education in sequential order;
  1. Take a pre-test to check your knowledge
  2. Module 1 – The Physiological Effects of Alcohol and Drug Use
  3. Module 2 – Understanding Blood Alcohol Content/ Alcohol’s Impact on Operating a Motor Vehicle/ Criminal Penalties for DUI Convictions/ Alternatives to Driving Impaired
  4. Module 3 – The Psychological and Sociological Effects of Alcohol and Drug Use
  5. Module 4 – Self-Assessment of Alcohol and Drug Use
  6. Module 5 – Substance Abuse Treatments/Resources Available in the Community for Substance Abuse Treatment/Self-Help Groups like Alcoholics Anonymous (AA), Narcotics Anonymous (NA) and Celebrate Recovery
  • At the end of each of the first four modules is a short quiz which will be answered in short sentences
  • Upon completing all five modules there is final exam. You are required to earn 70% on the final exam, but will have multiple attempts if needed
  • Certificate of completion will be available for download once you pass the final exam
  • Once you complete the online 16 hour DUI Education Arizona DUI Services will send a certificate of completion to the court or MVD depending on your situation

** Our 16 Hour DUI Education course will not be reported to the MVD or court in less two full days from the start of the course, regardless of how quickly you complete the course.**

**Your DUI education will need to be completed in 8 weeks from the start date of your first class**

DUI SCREENING

ALCOHOL/DRUG SCREENING PROCESS:

In order to successfully complete your DUI screening, you will need access to the internet and a smart device or computer equipped with a camera. Additionally, you must submit proof that you require a DUI screening by providing a copy of your citation, court documents, or an extended certified MVR. Lastly, a copy of a state-issued ID is also a mandatory requirement. Failure to provide the photo ID and proof of the need for a screening will cause delays in reporting to the courts and MVD.

 

**** Minors under 18 need court or case manager approval for DUI screening by Arizona DUI Services, as per ADHS policies.****

  • Pay for your Screening
  • Complete your profile
  • Schedule your screening
  • Email your ticket to [email protected] to confirm your BAC. ***Failure to provide confirmation of your BAC may delay the reporting of your screening to the MVD/Courts and a recommendation of 36 hours of DUI Treatment.***
  • Upload your state issued ID to your dashboard or email it to [email protected]
  • Complete your Alcohol and Drug Screening
  • The MVD and/or the appropriate court will be notified of your completed DUI Screening
  • Arizona DUI Services will email you a copy of the results of the screening and a copy of the MVD confirmation for your records
  • Per the discretion of our Clinical Director first time offenders with a BAC of .15 and higher will be recommended 36 hours of DUI Treatment
  • ***Anyone who misses two screening appointments will have to make another payment of $75 to schedule the third appointment***

8 HOUR SUBSTANCE ABUSE CLASS

8 Hour Substance Abuse Class process:

To successfully complete your 8 Hour Substance Abuse course, you will need an internet connection and a smart device/computer. Additionally, you must provide proof that you require the 8-hour course. This can be accomplished by submitting a copy of your screening note (if the screening was completed with another facility), or court documents. Lastly, a state-issued ID copy is also necessary. With a photo ID copy and proof of the need for the 8-hour course, reporting to the courts will be completed on time.

  4. Lecture 3- The Stages of Substance Abuse and Approaches to the Treatment of Substance Abuse
  5. Lecture 4 – The Psychological and Socioeconomical Effects of Drugs and Alcohol
  6. Final Exam
  • At the end of each module is a quiz.
  • Upon completing all four modules there is a final exam. You are required to earn 70% on the final exam, but will have multiple attempts to achieve 70%
  • Certificate of completion will be available for download once you pass your final exam

Once you have completed your 20 Hours of Treatment you will be emailed a certificate of completion. Additionally, if the MVD and/or the appropriate court needs confirmation of your completed treatment we will forward them a copy of your certificate of completion.

Upon completing the fifth module, there is a final exam. You are required to pass the final exam with a 70% to move into the next section. You will have multiple attempts at the final to achieve the passing 70%

Once you have completed all ten sessions one of Arizona DUI Services staff will follow up as an exit interview. The exit interview is designed to gauge your comprehension of the Supplemental course.

Once you have completed the course you will receive a certificate of completion. To ensure you are compliant with the courts your certificate of completion will be sent to the courts as well.
