Revocation Substance Abuse Evaluation

REVOCATION PROCESS:

Driving is a privilege! FOLLOW THESE STEPS AS DIRECTED!

1. Pull your EXTENDED- certified MVR at the MVD or at azmvdnow.gov and email in PDF format, ALONG with your completed DUI treatment certificates, and your picture ID to State Approved Evaluator- Robin Fernandez, at [email protected] Robin will review your MVR and DUI Treatment to confirm that you have completed the MVD requirements and are eligible for the Revocation Evaluation. IF YOU CANNOT PROVIDE A CERTIFICATE FOR COMPLETED HOURS OF DUI EDUCATION OR DUI TREATMENT, YOU WILL NEED TO COMPLETE A 36 HOUR DUI TREATMENT COURSE, PRIOR TO STARTING THE REVOCATION PROCESS, IF YOUR DUI IS AFTER 2011.
2. Upon MVR and DUI Treatment approval, Robin will email you a questionnaire form. Please reply to the email and respond to each question. Once the questionnaire is complete in the email and you are approved to move forward, Robin will send you a link to make your $125 payment. ONCE YOU HAVE PAID YOUR $125, PLEASE EMAIL ROBIN THIS PAYMENT UPDATE.
If you have THREE or more DUI’s and are not in remission, please know that Robin cannot move forward with you and this Revocation Evaluation process.
3. Once you have been approved to move forward, and have submitted your $125 payment, you will be sent a link to schedule your FINAL ZOOM appointment for your Substance Abuse Evaluation with Robin’s assistant, Cassia, ALONG with the REVOCATION application. You MUST complete the top of the application and send back to Robin in PDF, either through email or by mail.
4. Once your ZOOM Evaluation is complete, Robin will submit your application to the Driver Improvement team at ADOT MVD. You will then receive a copy by email as well.
YOU MUST FOLLOW THESE STEPS AND BE ELIGIBLE FOR THIS PROCESS IN ORDER FOR THE STATE APPROVED EVALUATOR TO SIGN YOUR APPLICATION AND SUBMIT TO DRIVER IMPROVEMENT.

16 HOUR DUI EDUCATION ONLINE

DUI EDUCATION PROCESS:

To complete your 16 hour DUI Education course, you will need an internet connection and a smart device/computer. You will also need to provide proof that you need the 16 hour course by submitting a copy of your screening note (if your screening was completed with another facility), MVD correspondence you are eligible for the interlock deferral program or court documents. Lastly, a copy of a state issued ID is required. Without the photo ID and proof you need to complete the Education, the reporting to the courts and/or MVD will be delayed until both items are provided.

  • Pay for the 16 Hour DUI course
  • Complete your user profile
  • If you completed a screening with an facility upload the Screener’s notes into your dashboard
  • Access your 16 Hour DUI Education through your user dashboard
  • Complete your online education in sequential order;
  1. Take a pre-test to check your knowledge
  2. Module 1 – The Physiological Effects of Alcohol and Drug Use
  3. Module 2 – Understanding Blood Alcohol Content/ Alcohol’s Impact on Operating a Motor Vehicle/ Criminal Penalties for DUI Convictions/ Alternatives to Driving Impaired
  4. Module 3 – The Psychological and Sociological Effects of Alcohol and Drug Use
  5. Module 4 – Self-Assessment of Alcohol and Drug Use
  6. Module 5 – Substance Abuse Treatments/Resources Available in the Community for Substance Abuse Treatment/Self-Help Groups like Alcoholics Anonymous (AA), Narcotics Anonymous (NA) and Celebrate Recovery
  • At the end of each of the first four modules is a short quiz which will be answered in short sentences
  • Upon completing all five modules there is final exam. You are required to earn 70% on the final exam, but will have multiple attempts if needed
  • Certificate of completion will be available for download once you pass your final exam
  • Once you complete the online 16 hour DUI Education Arizona DUI Services will send a certificate of completion to the court or MVD depending on your situation

**Your DUI education will need to be completed in 8 weeks from the start date of your first class**

DUI SCREENING

ALCOHOL/DRUG SCREENING PROCESS:

To complete your DUI Screening, you will need an internet connection and a smart device/computer with a camera function. You will also need to provide proof that you need a DUI Screening by submitting a copy of your citation, court documents or an extended certified MVR. Lastly, a copy of a state issued ID is required. Without the photo ID and proof that you need a screening the reporting to the courts and/or MVD will be delayed until both are provided.

****Arizona DUI Services does not offer DUI Screenings for minors under the age of 18****

  • Pay for your Screening
  • Complete your profile
  • Schedule your screening
  • Email your ticket to [email protected] to confirm your BAC. ***Failure to provide confirmation of your BAC may delay the reporting of your screening to the MVD/Courts and a recommendation of 36 hours of DUI Treatment.***
  • Upload your state issued ID to your dashboard or email it to [email protected]
  • Complete your Alcohol and Drug Screening
  • The MVD will be notified of your completed DUI Screening
  • Arizona DUI Services will email you a copy of the results of the screening and a copy of the MVD confirmation for your records
  • Per the discretion of our Clinical Director first time offenders with a BAC of .15 and higher will be recommended 36 hours of DUI Treatment
  • ***Anyone who misses two screening appointments will have to make another payment of $75 to schedule the third appointment***

8 HOUR SUBSTANCE ABUSE CLASS

8 Hour Substance Abuse Class process:

***Call the court and confirm they will accept Arizona DUI Services online 8 Hour Substance Abuse Class***

  • Pay for your 8 Hour Substance Abuse course
  • Complete your profile
  • Access your course through your user dashboard

Complete your 8 Hour Substance Abuse Class in sequential order

  1. Take a pre test to check your knowledge
  2. Lecture 1- AUD/SUD Self Assessment
  3. Lecture 2 – The Physiological Effects of Drugs and Alcohol
  4. Lecture 3- The Stages of Substance Abuse and Approaches to the Treatment of Substance Abuse
  5. Lecture 4 – The Psychological and Socioeconomical Effects of Drugs and Alcohol
  6. Final Exam
  • At the end of each module is a quiz.
  • Upon completing all four modules there is a final exam. You are required to earn 70% on the final exam, but will have multiple attempts to achieve 70%
  • Certificate of completion will be available for download once you pass your final exam

Once you have completed your 20 Hours of Treatment you will be emailed a certificate of completion. Additionally, if the MVD or the court you are working with needs confirmation of your completed treatment we will forward them a copy of your certificate of completion.

36 Hour DUI Treatment

36 Hour DUI Treatment process:

Lastly, a copy of a state issued ID is required. 56 Hour DUI Treatment

At the end of the first four modules there are assessment. You will answer the question with short sentences.

Upon completing the fifth module there is a final exam. You are required to pass the final exam with a 70% to move into the next section. You will have multiple attempts at the final to achieve the passing 70%

Once you have completed the 56 Hour DUI Treatment course and the exit interview you will be emailed a certificate of completion. Additionally, a copy of the certificate of completion will be forwarded to the court to confirm you have completed all of the recommended hours of treatment.

1:1 BHT Level Counseling

1:1 BHT Level Counseling process;

  • Pay for your counseling. When you are recommended multiple hours of counseling you can purchase all the hours at one time or on an hourly basis. Keep in mind when you purchase all the hours up front this allows Arizona DUI Services to schedule all your appointments at one time ensuring you meet court ordered time lines.
  • Once you pay for your hours someone on Arizona DUI Services staff will follow up with you to schedule your appointments.

Once you have completed all the recommended hours of counseling you will receive a certificate of completion and a copy will be sent to the courts as well.

Once you have completed the course and the exit interview you will receive a certificate of completion. To ensure you are compliant with the courts your certificate of completion will be sent to the courts as well.
