With 20 years of licensed independent substance abuse counseling, 10 years of teaching, and nine years of online education experience Arizona DUI Services is qualified to offer accommodating, affordable, and convenient DUI Alcohol and Drug Screenings, Revocation Evaluations,16 Hour DUI Education, and 20, 36 and 56 Hours of DUI Treatment to anyone anywhere in the state of Arizona. Arizona DUI Services is not a treatment facility, so there is no intake process saving you time and money. Arizona DUI Services will complete your DUI Screening online via Zoom, which connects you to your Screener. By completing the DUI Screening online, you can choose the day, time, and location to conduct your screening. To utilize our Alcohol and Drug Screening, Education or Treatment program, you will need a desktop, laptop, tablet or smart device with a camera function and an internet connection. Once you have completed your service(s) Arizona DUI Services will report it to the MVD, the appropriate court, and email you a copy of the MVD confirmation and your certificate of completion. Arizona Revocation Steps

Determine Eligibility for Arizona Revocation:

1. Send Extended Certified MVR

2. Send Photo ID

3. If you’ve had a DUI after 2010, send ALL completed treatment Certificates.

4. If you do not have proof of treatment, completion of 36hrs of DUI treatment is required to proceed

 

After Determination for Eligibility:

1. Complete AZ Revocation Application

2. Pay $125 to CK Counseling Group for Arizona Revocation Evaluation.

3. Complete Questionnaire

4. Complete Screeners/Consents in Portal

5. 30 min Evaluation for Further Screenings and Final Approval

 

Lastly, if you need an Ignition Interlock Device (IID) Arizona DUI Services can save you some money. We have an agreement with Intoxalock that entitles our clients to a free installation and your first month free. 16 HOUR DUI EDUCATION ONLINE

Level One and Level Two DUIs require 16 hours of Education. Our online 16-hour DUI Education course meets the Arizona Department of Health Services and MVD requirements. Our online option allows you to complete your court-ordered Education at your convenience with 24-hour access to your classes.

To complete your 16-hour DUI Education course, you will need an internet connection and a smart device/computer. You will also need proof that you need the 16-hour course by submitting a copy of your screening note (if another facility completed your screening) and MVD correspondence; you are eligible for the interlock deferral program or court documents. Lastly, a copy of a state-issued ID is required. Without the photo ID and proof you need to complete the Education, reporting to the courts and/or MVD will be delayed until Arizona DUI Service’s have both items.

DUI SCREENING

After being arrested under the suspicion of a DUI, the MVD requires a DUI Screening (ARS 28-1385) to meet state requirements. The DUI Screening is an assessment tool used to determine hours of education, treatment or both you will be required to complete. The screening results and recommended education hours, treatment, or both are sent to the courts. Once the courts have the recommended hours, they expect the offender to complete them.

Arizona DUI Services will conduct your screening via Zoom which connects you to your Screener in a video conference room.

To complete your DUI Screening, you will need an internet connection and a smart device/computer with a camera function. You will also need to provide proof that you need a DUI Screening by submitting a copy of your citation, court documents or an extended certified MVR. Lastly, a copy of a state issued ID is required. Without the photo ID and proof that you need a screening the reporting to the courts and/or MVD will be delayed until both are provided.

Screenings with Arizona DUI Services are valid for 12 months.

20 HOUR DUI TREATMENT

Level 1 DUI Offenders may be required to complete 20 Hours of DUI Treatment by the MVD or the courts. Our 20 Hour Treatment meets the requirements of the Arizona Department of Health Services and MVD. Our 20-hour treatment option allows you to complete your court-ordered education at your convenience with 24-hour access to the program. To complete your 20-hour DUI Treatment, you will need an internet connection and a smart device or laptop.

To complete your 20-hour DUI Treatment course, you will need an internet connection and a smart device/computer. You will also need proof of the 20-hour course by submitting a copy of your screening note (if another facility completed your screening) or court documents. Lastly, a copy of a state-issued ID is required. Without the photo ID and proof, you need to complete the Treatment, reporting to the courts and/or MVD will be delayed until Arizona DUI Services has both items. Once you complete the 20-treatment course, Arizona DUI Services will send you a certificate of completion. Please be advised that you are expected to work through the worksheets independently.

36 HOUR DUI TREATMENT

When you complete a DUI Screening, there will be recommended hours of education, Treatment, or both. When determined per Arizona Department of Health Service guidelines that you are a level 1 offender, you will be required to complete 36 hours of DUI Treatment. The 36-hour Treatment course consists of 16 hours of DUI Education and 20 hours of Treatment. At Arizona DUI Services, your 36-hour Treatment course is completed entirely online, and with 24-hour access to the content, you complete the course when you have time, making it convenient for our busy clients.

To complete your 36-hour DUI Treatment course, you will need an internet connection and a smart device/computer. You will also need proof of the 36-hour course by submitting a copy of your screening note (if another facility completed your screening) and court documents. Lastly, a copy of a state-issued ID is required. With the photo ID and screening note, Arizona DUI Services reporting will be timely.

Arizona DUI Services emails you a certificate after you finish the coursework. However, you are expected to work through the worksheets on your own. If the MVD or the court you are working with needs confirmation of your completed treatment, we will forward them a copy of your certificate of completion.

56 Hour DUI Treatment

Your DUI Screening is an assessment tool for determining Level 2 and Level 1 offenders. Once you complete your screening, you will have a recommendation for education and treatment hours. Level 1 DUI offenders can be asked to complete 56 hours of DUI treatment. 56-hour DUI Treatment programs consist of 16 hours of DUI Education and 40 hours of DUI Treatment. Arizona DUI Services 56-Hour DUI Treatment is court-accepted and MVD-approved. Arizona DUI Services 56-Hour DUI Treatment is a self-paced program completed online, and with 24-hour access, our course is convenient.

To complete your 56-hour DUI Treatment course, you will need an internet connection and a smart device/computer. Be advised it is expected you complete the worksheets independently. You will also need proof that you need the 56-hour course by submitting a copy of your screening note (if your screening was completed with another facility) and court documents.
